You are providing consent for the First Point of Contact Team to get in touch with you through your preferred method of contact.
If you are completing this form for yourself By submitting information in this online form you are providing consent for this information to be shared between City of Cardiff Council Officers and relevant agencies and partners. Services are delivered by teams of professionals. It will be necessary for other members of the team, who may be involved in your care, to have access to information about you, on a need to know basis. This will include Health & Social Services Professionals from the City of Cardiff Council and Cardiff and the Vale University Health Board. It could also include named family members or named organisations / bodies / workers / care agencies who may be able to help and support your needs. This will help them understand your needs and avoid having to repeat some parts of the assessment or review. Information may also be shared with other organisations including those that provide care services and those that inspect and monitor our work such as the Care and Social Services Inspectorate Wales and organisations such as the NHS who work with us to provide services. You can withdraw your consent at any time, by writing to or informing your care co-ordinator or assessor.
